Submission Instructions
Completing your paperwork is simple. Follow these few steps to ensure everything is ready for your child’s evaluation.

Complete & Sign: You can fill them out digitally or print them at home. Please ensure legal guardians sign each document where indicated.
Submit Your Files: Upload completed forms through our secure portal, email them to our clinic, or simply bring printed copies directly to your first appointment.
Timing Expectations
To ensure our clinical team has time to review your profile, please submit all documents at least 48 hours before your first scheduled session.
